Requests To Backdate Progress Notes Access

Requests to backdate progress notes in AWARDS beyond the permitted two weeks, must be approved and requested from Management staff (RM/ARM) .  This request can be made by email to Lenny Truby or Mark Wasiewicz.

Recently we receive an inquiry from a manager about consumers who do not have an emergency contact or next of kin and how to enter this information into AWARDS.

Emergency contact or next of kin contact information is located in the profile module under contacts.
Census>Profile>Face Sheet>Choose Consumer>Scroll down to "Current Contacts Info:"

Upon review and consideration, all future/existing consumers should have a Next of Kin and Emergency Contact information entered into AWARDS. If a consumer doesn't have a next of kin and/or emergency contact enter "NONE" in the first AND last name fields.

These fields should be completed/review at the time of admission to eliminate any confusion among staff members. If this field is blank one may think information was never entered OR one may think since the field is blank no emergency or next of kin exists when actually there is a contact person.

AWARDS Update 10/29/2014

Beginning Monday, November 3rd marks the start of the AWARDS implementation throughout STEL's housing services. On this date the following tasks will now be entered into AWARDS:
  • Admission Notes
  • Progress Notes
  • Service Plans
  • Service Plan Reviews
  • Discharge Summaries
Additional functions and tasks will be rolled out throughout the the next few months.

The Implementation Team would like to thank everyone in advance for their cooperation and effort as we move forward toward an electronic health record (EHR). AWARDS will allow us to improve our regulatory compliance obligations, reduce data input/reporting redundancy, ability to collect and report consumer information, and improve satisfaction and service. 

Please use the online "Suggestion Box" located under the SPEAK UP! menu for suggestions or concerns about AWARDS. 

Data Entry into AWARDS

May 8th assigned staff members will begin data entry into AWARDS. STEL's AWARDS platform is now ready for consumer demographic information to entered. AWARDS assessment and services components are currently being customize by our provider, Foothold Technologies. Each program should enter a minimum of  consumers into AWARDS.
