Dear Employee,

Throughout our recent informational meetings regarding the new Univera Healthcare insurance plan we discussed preventive care services being covered in full. On your benefit summary from Univera, a routine vision exam is listed as a preventive care service, however, this is a preventive care service that is not covered in full as most other preventive care services are.

Although Univera considers this a preventive care service, they do not cover it in full, this service is subject to the deductible. I do advise that, if you use a participating Univera eye care provider, you give the provider your Univera identification card so that they can submit the claim to Univera for you and you may be entitled to network discounts.

I apologize that it was indicated that the routine vision exam would be treated as a preventive care service.

If you have any questions, please do not hesitate to contact me at (716) 487-4760.

Thank you,

Krystal Waite
