Attached is the new Direct Deposit Form with an HSA deposit option. Effective immediately staff members must use this this form for ALL future new and modified direct deposit instructions. Attention All High Deductible Medical Plan Participants w/ HSA HSA participants MUST submit a direct deposit form regardless if the participant voluntarily elects to deposit additional dollars beyond STEL's seed money. The direct deposit form will instruct the payroll department where to deposit the seed money.
