ATTENTION ALL STAFF MEMBERS ACTION IS REQUIRED

Notice and Acknowledgement of Pay Rate and Payday

Under Section 195 of New York State Labor Law, as amended by the Act, requires employers provide notice to employees of their rate(s) of pay, designated pay day, the employer’s intent to claim allowances (like tip or meal allowances) as part of the minimum wage, and the basis of wage payment  (whether paying by hour, shift, day, week, piece, etc.).

To assist us to comply with this law, please perform the following steps:
  1. Review and sign the white copy.
  2. Place the signed white copy in the return envelope.
  3. Place the envelope in the inner office mail to return Human Resources no later than Friday, February 3, 2012.
  4. Retain the yellow copy for your records.
