We'd like you to receive your Dental and Vision Claim reimbursements in a timely and efficient manner. In order to do so, we have developed a dental and vision procedure outlining the reimbursement process for you to review. If you need a Dental or Vision claim form, they can be found on this site in the HR Forms. Feel free to contact Mark or Sandy in the HR Department if you have any questions.
