We have partnered with Blue Cross Blue Shield to conduct an employee wellness interest survey to help us identify wellness needs and plan for future programs/activities. The objective of STEL's wellness program is to 1) have a healthy employees and family members and 2) to assist in controlling future healthcare costs.
The Blue Cross BlueShield employee interest survey is available online and is completely confidential and anonymous. The survey asks some questions regarding current health habits. It will also gather information on what programs your employees are interested in and if they are willing to participate. Blue Cross Blue Shield will analyze the data and provide us with an aggregate report, which will be used to plan future wellness programs.
Here are the instructions on how to implement the survey: Please take several minutes to complete a confidential and anonymous employee wellness interest survey from Blue Cross BlueShield so that we may learn more about your health & wellness needs and interests. Your responses will be used in planning future wellness programs. Participation in the survey is voluntary.
The information you provide on this survey will be kept strictly confidential. Federal and state laws mandate the privacy of your protected health information (PHI).

April is Life & Disability Enrollment Month ! ! !
The month of April is the open enrollment period for new voluntary life and disability insurance policies for eligible staff members.
This is the time for: - Eligible staff members may enroll in the voluntary life and/or disability insurance.
- Existing participants may review or change coverage i.e. add dependents, change insurance amounts etc.
All new policies will be effective May 1st, 2012.
Life & ADD Insurance offered through Lincoln Financial Group - Employee can purchase life insurance in $10,000 increments
- Spousal coverage is available in $5,000 increments
- Dependent coverage (6 months to 19 years old) $10,000
- Premiums are based on age and insured amount
Supplemental Short Term Disability offered through UNUM - Employee can purchase disability insurance and is able to choose the length of coverage, waiting period, and insured amount. Premiums are determined by plan design, age, and insured amount.
Enrollment & Education Sessions The education and enrollment sessions will last about 20 minutes. Staff members will have an opportunity after to meet privately with a Veracity representative to determine the appropriate plan design and costs. - April 17th, 1:00 PM at 97 Forest Ave. Jamestown, NY
- April 18th 1:00 PM at 715 Central Ave.
- April 19th 1:00 PM at Perkins Hall Community Residence
- April 25th 10:30 AM at Olean Community Residence

As of this moment there are no appointments scheduled by staff members to meet with a Courier Capital Representative for either Wednesday, March 21st or Thursday, March 22nd.
Ed Miner, Courier Capital Rep will STILL be on site at 97 Forest Ave. Office on Wednesday from 1:00 PM to 2:00 PM for any staff member to stop by for any retirement questions or advise.
As of right now we will cancel the Thursday, March 22nd meeting date. |

Courier Capital representatives will be available for individual meetings with staff members to discuss STEL's Employer Contribution Retirement Plan (profit sharing plan) and 403b(7) plan options.
What's the difference between STEL's Employer Contribution Plan and 403b(7) Plan? - The STEL Employer Contribution Retirement Plan is the plan where STEL contributes monies to your account administrated by P&A Administrative Services. The 403b(7) retirement plan accounts allow staff members to set aside monies for retirement. Courier Capital representatives can distinguish the plan differences for you.
Why should I meet with a Courier Capital representative? - Non-participants may sign up to speak with a retirement plan consultant to set retirement goals, ask questions and evaluate plan participation and options.
- New plan participants can set retirement goals, enroll in either plan, complete beneficiary forms, and select investment options.
- Existing plan participants can review retirement goals, investment options, amend beneficiary forms, and get answers to your retirement questions.

AIDS Community Services of Western New York will be presenting an informational session for any staff interested. The training has been rescheduled to Wednesday, March 14 at 1 p.m. at Perkins Hall. The session will last about 2 hours. Anyone who would like to attend can email Sue at chiappones@stel.org or call the Gowanda CR so we have enough materials to distribute. |

Payday
Under Section 195 of New York State Labor Law, as amended by the Act, requires employers provide notice to employees of their rate(s) of pay, designated pay day, the employer’s intent to claim allowances (like tip or meal allowances) as part of the minimum wage, and the basis of wage payment (whether paying by hour, shift, day, week, piece, etc.).
To assist us to comply with this law, please perform the following steps: - Review and sign the white copy.
- Place the signed white copy in the return envelope.
- Place the envelope in the inner office mail to return Human Resources no later than Friday, February 3, 2012.
- Retain the yellow copy for your records.
